With advanced training and years of experience, Dr. Caballar-Gonzaga provides comprehensive evaluations and treatments for sleep disorders in both adult and pediatric patients. PUBLIC INFORMATION IS HEREBY GIVEN THAT: Effective October 11, 2025, Flor Caballar-Gonzaga, MD, will no longer provide Sleep Disorder Services at RMCHCS.
After October 11, 2025, medical records will be maintained at RMCHCS, and with written patient authorization, a copy of your medical record can be obtained from the RMCHCS H.I.M. Department at 1901 Red Rock Drive, Gallup, NM 87301.
When needed, RMCHCS will be glad to assist in establishing care with a new provider.
