Job Title: Community Health Worker

Under the direction of a Registered Nurse or Emergency Room Tech, the Community Health Worker performs SBIRT (Screening, Brief Intervention, and Referral to Treatment) Healthy lifestyle questionnaires on all patients 18 years of age or older. This is accomplished through criteria outlined on the core competency evaluation form. The Community Health Worker is responsible for completing all duties as assigned.
